Cancellation Form (Child's name for first and last name)

I acknowledge that I am aware of and agree to the 30-day notice requirement for cancellation.

All memberships are billed on the 1st of each month. Cancellation requests must be submitted on or before the 1st of the month to avoid being charged for the following month.

I also acknowledge that all memberships require a minimum three-month commitment. The three-month commitment begins on the date of the initial billing and includes the initial billing date, followed by two additional consecutive monthly payments billed on the 1st of each month moving forward.
